What kind of shells was he shooting?
I dont know anything about the IWI Tavor TS12, but some hunting shotguns need to be broken in with a few boxes of high brass shotshells before they will reliably cycle low brass.

At least twenty boxes are through it, not sure on the shot size but also slugs.

Twenty boxes and still problems, I'd be reaching out to IWI. This is a combat shotgun in use by Israeli military. It is supposed to work so if it doesn't, something is wrong.

"Right out of the box without any extra care, we tested this out in the field on a rainy day. We basically experienced a 1 out of 25 issue with the shell catching in the ejection port. However, after cleaning it and taking it out again and again, it has fired much better with a ~2 out of 100 catching, which I think is much more acceptable for a semi-automatic shotgun firing at the rate I do, and at this price. I’ve recently updated this article since I’ve now had it for over 2 years, and it has performed even better over time. I therefore, think this needs good cleaning and lube in order to get in working order. Now you might say that this could be an isolated incident, but I’ve talked to a couple of other TS12 owners and they have said the same thing."

Do a quick clean-up on a brand new TS12..... to get rid of any factory protective packing grease or oil. Here is a link to the disassembly and reassembly instructions.

Akso, as per the TS12 user manual, use 2-3/4" or 3" shells
with a minimum velocity of 1200fps and a minimum weight of 1-1/8oz
.
I followed the above instructions, and my TS12 never had a malfunction.
It is a fun shotgun to shot, enjoy!
